Original photo ID
Bring valid, original government-issued photo identification. Photos and photocopies of identification are not accepted.
The complete document
Bring the complete document, including every page and attachment. For a certified true copy, bring the original.
Wait to sign
Do not sign beforehand unless I have confirmed otherwise. Each person whose signature must be witnessed must attend in person.
Recipient instructions
Bring any instructions provided by the receiving organization, particularly for documents intended for use outside Canada.

What is an affidavit?+
An affidavit is a written statement of facts that a person swears or affirms is true. The document must be prepared before the appointment.
What is a statutory declaration?+
A statutory declaration is a written statement solemnly declared to be true. It is often used where no specific affidavit form is required.
What is a certified true copy?+
A Notary Public compares a copy with the original document presented and certifies that the copy is a true copy of that original.
What is an apostille?+
An apostille is a certificate issued by a designated Canadian authority for certain public documents intended for use in another country. Requirements depend on the document, issuing jurisdiction and destination.

Yes. All services are by appointment. Mobile and after-hours appointments may be available within Sylvan Lake.

No. Documents must be complete before the appointment. I do not prepare, revise or provide legal advice about documents.

Could an appointment be refused?+
Yes. I may be unable to proceed if identity, understanding or willingness cannot be confirmed; the document is incomplete; the request is outside my authority; or there are concerns about fraud or another unlawful purpose.
